Reordering Items on Published Forms

After creating forms using a form template, you can reorder form items on individual forms. This applies to published Supervisor Forms, Field Notes, and Procedure Forms.

  1. Open the Form Template that you used to build the form of interest.

  2. Click on the title of the form on which you wish to re-order items.

  3. Click and drag on the “cross” icon to re-order the assessment items.

  4. The changes will be saved automatically. This must be done for each individual form that you wish to re-order.
